Insurance for Providers:
You are responsible for presenting your insurance card at time of service. If your insurance requires a co-payment, or if your deductible has not been met, this amount is due at time of service. We will submit in-network claims for you. We will not submit claims to out-of-network insurance companies. However, we will not negotiate claims with your insurance company. Superbills can be provided via email after the visit has be paid in full and a charge slip has been created.
