New Patient Process
Please contact our office at 419-931-3020 to inquire about scheduling a New Patient appointment.
Once scheduled, our front office staff will email you a link for the Patient Portal where you will complete the New Patient Intake Forms before your first appointment. No need to print these forms. This is all done electronically and submitted directly to your chart once completed.
Portal Setup
Once scheduled with our office, setting up your Patient Portal is easy.
First, you will need to register for a new account by providing the First Name, Last Name, Date of Birth and Email for the client scheduled. It is very important that these items match the information we have on file. Once a portal account is set up, you will be able look at your scheduled appointments, complete forms, pay outstanding balances, request prescription refills and direct message our office.
